New Patient Information 

 

Dr. Pamela Maragliano, along with the team at Salem Dental Arts, would like to extend a warm greeting to you and thank you for choosing our services. At Salem Dental Arts, we strive to provide the highest level of quality care to achieve optimal oral health for our patients.  Dr. Maragliano has extensive training in all facets of prosthodontic, as well as preventive care. 

 

We currently see patients routinely on Monday, Tuesday, Wednesday and Thursday.  Alternative days are available for complex cases.  In addition, Dr. Maragliano is on call for all patients of record and can be reached after hours or weekend in the event of an emergency. 

 

What to bring with you:

• Please bring completed patient forms to your appointment.

• Your dental insurance card so that we may make a copy for our records.

• We require payment at the time of service. We accept cash, check and credit cards. Please bring your payment with you to your appointment. If you have questions regarding insurance coverage, please call our office. 

• Previous dental records/X-rays.

• List of all medications you are taking including name, strength and how many times a day it is taken.
